Nursing Opportunities

Primary Home Care RN for Hospice Patients to serve as the care manager, providing, arranging for, teaching and supervising the health care of patients in the home for hospice patients.

Full time , 8 a.m. – 4:30 p.m. with on-call rotation.  Must be licensed RN with the State of Indiana.  Minimum 1 year experience in a hospital setting, nursing home, ECF, or home care. Previous hospice experience preferred.

 

Primary Home Care RN for Hospice Patients to serve as the care manager, providing, arranging for, teaching and supervising the health care of patients in the home for hospice patients.

 Full time , 8 a.m. – 4:30 p.m. with on-call rotation.  Must be licensed RN with the State of Indiana.  Minimum 1 year experience in a hospital setting, nursing home, ECF, or home care. Previous hospice experience preferred.

Wound Ostomy Care Registered Nurse to provide care and support for our patients with wound, ostomy, and continence care needs.  Will provide comprehensive assessments and develop plan of care including goals, implementation, and evaluation of outcomes for patients.  The WOCN will serve as a clinical expert and resource for wound, ostomy, and continence care.  The WOCN will provide training and education to our interdisciplinary team and serve as a consultant for complex wound, ostomy, and continence clients.  This position will collaborate with our marketing team to help educate the community and referral sources of our specialty services.

Full and part time positions available.  8 a.m. – 4:30 p.m. with weekend or on-call rotation.  Must be certified as a WOCN and licensed as an RN with the State of Indiana. Minimum 1 year experience in a hospital setting, nursing home, ECF, or home care caring for clients with abdominal stomas, drains, wounds, fistulas, pressure ulcers, and incontinence problems.

Patient Care Opportunities

Therapists:  Physical, Occupational, and Speech Therapists to provide therapy in accordance with the physician's written orders in the patient's home.

Full-time or part-time positions available.  Must be a Licensed Therapist in the state of Indiana.

 

Home Health/Hospice Aide to provide personal healthcare for individuals in their residence or at the VNA Hospice Center under the supervision of a registered nurse.

Full, part-time and PRN (as needed) positions working in the field and the Hospice Center plus participating in on-call rotation.

 

Other Opportunities

Hospice Social Worker to work in conjunction with the Hospice interdisciplinary group (IDT) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel.  Some of the responsibilities of this position include: receive referral and initial physician orders from intake or skilled nurse; evaluate psycho/social, emotional and spiritual factors relating to the patient's terminal illness and its effect on the family; provide input to the coordinated plan of treatment for Hospice patients; implement, revise, and evaluate the social service aspects of the patient’s plan of treatment on an ongoing process; provide social services including short-term individual counseling, crisis intervention, assistance in providing information and preparation of advance directives, funeral planning, issues involving transfer of fiscal, legal and health care responsibility; participate in bereavement program duties as assigned; and attend IDT meetings as scheduled and initiates patient case conferences as appropriate.  Hospice Social Worker will be cross-trained to make home hospice visits.

Full time, primarily 8 a.m. – 4:30 p.m., must be flexible with schedule to accommodate patient and family needs. Participate in on-call rotation.  Master of Social Work degree from a school of social work accredited by the Council on Social Work Education. Minimum of one year of social work experience in a healthcare setting.  Must have and maintain a current social worker license with the State of Indiana.  Demonstrate knowledge of Hospice philosophy, concepts, and bereavement counseling.  Exhibits sensitivity to issues of loss and grief. Must hold and maintain a valid driver’s license and auto insurance.

Hospital Liaison  The Hospital Liaison is responsible for developing relationships at the designated hospitals and serving as a resource to the designated hospitals to increase awareness of the agency, increase active client count, develop referral sources with physicians and hospital staff, coordinate with intake to identify needed home care and hospice services for patients in all stages of life.  The hospital liaison will primarily function as an on-sight Intake Coordinator.

Part-time working between the hours of 9:30 a.m. and 6 p.m. with weekend rotation.  Some of the qualifications include: Registered Nurse with current IN license. Two years experience in home care and a Baccalaureate degree in nursing preferred.  Prior experience in case management, discharge planning and community/physician relations preferred. Previous marketing experience with a track record of success encompassing customer service, community relations, and physician relations preferred. Ability to facilitate cooperation and communication among team members effectively. Ability to work independently, exercising appropriate judgment at all times. Demonstrates strong communication, organizational, and interpersonal skills. Ability to deal effectively in stressful situations; demonstrates good conflict resolution skills.  Ability to think quickly. Demonstrates knowledge and ability to use the computer (including laptop) for clinical record documentation and communication.  Must be computer literate.  Strong data entry skills preferred.

Community Liaison  is responsible for increasing public awareness of the agency, increasing active client count, developing referral sources, representing the agency in community events and coordinating with intake to identify needed home care and hospice services for patients in all stages of life.  Provides direct education regarding home care and hospice services to referral sources, physicians, and the community. Serves as a resource for staff. Performs other duties as assigned.

Part-time working between the hours of 8 a.m. and 4:30 p.m.  Some of the qualifications include: Associates or bachelors degree preferred.  Two years marketing experience preferred.  Track record of success encompassing customer service, community relations, and physician relations preferred.  Excellent presentation skills. Ability to speak clearly/effectively and engage with small and large groups.  Ability to work independently, exercising appropriate judgment at all times.  Demonstrates strong interpersonal and written and oral communication skills. Ability to deal effectively in stressful situations; demonstrates good conflict resolution skills.  Ability to think quickly. Must be computer literate.  Ability to use laptop and Power Point required (must become proficient in both within first 60 days).   Must have and maintain current driver’s license and automobile insurance.

 Float Team Assistant is responsible for providing back-up coverage for the skilled and hospice Team Assistants, Team Clerical Associates, Intake Clerical Associates, and other clerical positions within the agency.  The Float Team Assistant will also be responsible for maintaining the on-call schedules for professional and paraprofessional staff and for maintaining clinical supplies for the agency.  The Float Team Assistant’s role is crucial to the continuing functioning of the teams.

Full-time position working flexible shifts between 7 a.m. - 7 p.m. (7-3:30 and 10:30-7) Monday-Friday, and participate in weekend rotation 7:30 a.m. – 4 p.m. (approx every 5 weeks).  Some of the qualifications include: High School diploma or GED equivalent.  Minimum two years office experience, preferably in a health care environment.  Previous scheduling experience preferred.  Proficient in word processing, data entry skills, and have excellent computer skills.  Knowledge of general office procedures and medical terminology preferred.  Excellent organizational, communication, and critical thinking skills.  Ability to prioritize workload, handle multiple tasks, and follow-through on assignments.  Excellent customer service skills.  Ability to perform work accurately and with attention to detail.  Ability to develop effective working relationships with all staff members. Must have and maintain current driver’s license and automobile insurance.
