Home health care
encompasses a broad spectrum of health and social services that can be
delivered to recovering, disabled, or chronically ill persons in the
home environment. These services include the traditional core of
professional nursing and home health aide services, as well as hospice
care, physical therapy, speech therapy, occupational therapy, Meals on
Wheels, Lifeline, companion homemaker, medical social services and
nutritional services. All of these services are provided locally
through the Visiting Nurse Association of Porter County.

The
mission of the Visiting Nurse Association is to provide quality
health care services in an effective, innovative and personalized
manner to members of the community, within the resources available,
enabling them to preserve their dignity and maintain their highest
level of independence while remaining in their homes.
