The mission of the Visiting Nurse Association is to provide compassionate home and hospice care in an innovative and personalized manner to those in need, within the resources available, enabling them to preserve their dignity and maintain their highest level of independence.

 

Planned Giving

When you include a planned gift to the Visiting Nurse Association Foundation in your retirement and estate plans, you help the Visiting Nurse Association fulfill its mission and create a legacy of caring that will extend well beyond your lifetime.

 

Planned giving is a powerful and meaningful way to give back - to leave a philanthropic legacy for your community and the charities about which you care so much.

Bequests and Wills

Charitable bequests to the Visiting Nurse Association Foundation, Inc. support the programs and services provided by the Visiting Nurse Association of Porter County Indiana, Inc., including skilled nursing, therapy, hospice care and Meals on Wheels. These donations enable the Visiting Nurse Association to continue to provide quality care for patients and their families.

The following are examples of the type of simple language that may be used in your will

to make a bequest to the Visiting Nurse Association Foundation, Inc. They are intended only

as examples and must be tailored to meet your specific needs and desires.

  • Legal Name of the Corporation: Visiting Nurse Association Foundation, Inc.

  • Federal Tax ID Number: 35-1655711

  • GENERAL BEQUEST: I give, devise and bequeath to The Visiting Nurse Association Foundation, Inc., a corporation existing under the laws of the State of Indiana and located in the City of Valparaiso, Indiana ________(fill in a percentage of your estate, a certain sum of dollars or a description of particular property) to be used at the discretion of The Visiting Nurse Association Foundation, Inc.

    SPECIFIC BEQUEST: I give, devise and bequeath to The Visiting Nurse Association Foundation, Inc., a corporation existing under the laws of the State of Indiana and located in the City of Valparaiso, Indiana ________(fill in a percentage of your estate, a certain sum of dollars or a description of particular property) and direct that it shall be used for the following purpose(s):________(in detail, specify the purpose(s) you wish to support with your bequest). If at any time the need of income for such purpose ceases to exist, The Visiting Nurse Association Foundation, Inc. shall be and is hereby authorized to use this bequest for such purposes as in the judgment of the Foundation shall best promote the welfare of the Foundation and the spirit of its benefactors.

    NOTE: SAFEGUARDING RESTRICTED FUNDS: If the original purpose identified in a specific bequest no longer exists, the bequest may not be granted to the Foundation. For example, a specific bequest of property to support a special program at the Foundation may not be granted if, at the time the will is probated, that program is no longer in existence. The last sentence in the above specific bequest form safeguards against this possibility.

    RESIDUAL BEQUEST: All the rest, residue and remainder of my estate, both real and personal, wherever located, I give, devise and bequeath to The Visiting Nurse Association Foundation, Inc., a corporation existing under the laws of the State of Indiana and located in the City of Valparaiso, Indiana.

     

    Additional Planned Giving Options

    Appreciated Securities are an attractive gift for people who have held stocks or bonds for a long period of time, especially when the appreciation is so great that the capital gains tax would take a significant portion of the proceeds upon liquidation. Gifts of appreciated securities enable you to make a larger contributions to the Visiting Nurse Association Foundation, Inc. at reduced cost to you, thereby benefitting you, the programs and services of the Visiting Nurse Association and ultimately the community.

     

    Life Insurance policies that you no longer need can make a significant gift while you receive a current income tax deduction for the cash value and any premiums you pay in the future. In addition, you may choose to add the Visiting Nurse Association of Porter County as a beneficiary of a life insurance policy, designating any percentage or portion of the benefit you would like.
     

    Retirement Plan Assets may be a particularly effective way to fund a charitable bequest. Depending on the size of your estate, income and estate taxes may consume a large portion of this asset. Naming the Visiting Nurse Association Foundation, Inc. as the beneficiary for all or some of the assets remaining in your plan at your death can substantially reduce or eliminate your tax liability.

     

    Porter County Community Foundation

    The Visiting Nurse Association works in cooperation with the Porter County Community Foundation to assist donors who wish to support the organization with a gift annuity, charitable remainder trust, charitable lead trust or endowment gift.

     

    Donations or bequests may be made to the following endowment funds held by Porter County Community Foundation to support the programs and services of the Visiting Nurse Association:

    Visiting Nurse Association Foundation Fund

    Visiting Nurse Association Hospice Fund


    Gift Annuities are a simple way to make a charitable gift. Cash or appreciated securities are given to support the Visiting Nurse Association Foundation, Inc. and the donor receives income for life. In addition to an immediate tax deduction, a portion of the income received is tax free.

    Charitable Remainder Trusts offer a triple advantage. You receive an immediate tax deduction for a charitable gift; the assets are removed from your estate avoiding future estate taxes; and you, or someone named by you, can receive a lifetime of income from your gift.

    Charitable Lead Trusts can be established naming the Visiting Nurse Association Foundation, Inc. as beneficiary of a specified percentage of income from a trust for a specific number of years. Ultimately, the residual value of the trust reverts to other named beneficiaries, such as children or grandchildren.
